POOL FACILITY ACKNOWLEDGMENT AND WAIVER AGREEMENT

INSTRUCTIONS

1)    Thoroughly read and review the agreement.

2)    If you agree with the pool facility terms, please fill-in the date and property address information, where indicated on page 1.

3)    Date, print and sign the owner information, where indicated on page 2.  ALL DEEDED PROPERTY OWNERS MUST DATE AND SIGN THE AGREEMENT at the bottom of page 2.  Do NOT have any other individual, including tenant(s), sign the agreement as the property OWNER is legally responsible for the tenant(s).

4)    Please do not write in the box that is marked “For Office use only.”   When the pool key card is issued, the serial number on the card will be written in this area, along with the date of issue – This box is for CMHOA use only.

5)    Do not cut, tear or alter the form in any way.  Return both pages in their entirety.  

 

Once your form is completed and received by CMHOA, a pool key card along with a duplicate copy of the agreement, will be mailed/delivered to you.  Generally, pool key cards are only issued during open months of the swim season.

Pool key card shall only be issued to property owners.  If you wish your pool key card to be issued to a property manager on your behalf, please provide a written letter noting the individual’s name, telephone number, and address.   Pool key cards may not be issued to Tenants.
